Default Noobie question about memory addresses

I have a disassembled 65c816 ROM and was wondering how to translate the:
$PG/ADDR to the physical file location. Just running a search I found that:
$01/818A corresponds to 838A in the file
$04/8107 corresponds to 20307 in the file
$14/FF0A corresponds to A810A in the file
and a few others. What method would one use to translate the memory page/address to the data's physical location in the ROM file? Thanks!

(Page * $8000) + Addr - $8000 + $200

don't add the $200 if there's no header in the ROM.

it's excellent knowing the way to calculate the address, but this program makes it easier

One last question: What about addresses like 7F:xxxx? The equasion doesn't give correct values for those like the program mentioned does. Are there conditions where the equasion changes?

That's RAM.
