No you don't. It will add features like the 'real time save' (savestate-like) save feature, or reseting to the menu (or fixing save problems that some games may have), but many games actually will work ok without patching.

I should add also that you would need to make a gamename.dat (ie, if the rom is Awesome Mario Bros Game.gba, the DAT file should be "Awesome Mario Bros Game.dat") file in the GAMESAVE directory so it will save the save to a file on rebooting the unit. You can just copy an existing one, also there should be one in the M3 Game Manager's directory that is blank.
