I had the same problem with Lunar: The Silver Star while using an ISO+MP3 image. Turns out whoever encoded them to MP3s ended up adding an extra second or so of silence at the start. I tried fixing it, but without a frame of reference, it was so hard I gave up. I've resolved to only use BIN (optionally with a CUE file) images now since they don't have this problem.
Incidentally, Kega Fusion (which is now regarded as having more accurate MegaDrive/Genesis, MegaCD/SegaCD and 32X emulation than Gens) only accepts BIN images. This situation won't change, so don't bother asking SteveSnake.

You're talking about the European MegaCD BIOSes and games, so the answer is yes, they're PAL so 50FPS is perfectly normal. However, you'll get 60FPS with either Japanese MegaCD BIOSes and games or SegaCD BIOSes and games.
