You need to place the paddles to the correct spots on the person's anatomy and then press A,B,B,A,C,A,B, Start.

I suppose it's a moot point anyway, being that there's no C button in the picture the poor guy wouldn't make it anyway ;> Besides, I would have thought the old-standard Contra 30-lives code would have been more appropriate.
